Clinical Documentation

The output layer of Clinical AI: structured, specialty-appropriate reports ready for clinician review and sign-off.

What clinical documentation means at MedTalk

Clinical documentation is the output layer of Clinical AI: the structured report a clinician actually reviews, edits and signs off on. Ambient capture produces the raw material; clinical documentation is the structured, specialty-appropriate record it becomes.

Report types MedTalk generates

  • SOAP and specialty-specific documentation
  • Referral and consultation letters
  • Patient summaries and handover documentation
  • Custom clinical templates, created or refined with natural-language prompts
  • Multilingual documentation: consult in one language, generate the report in another

From transcription to signed-off record

The transcription and generated report are shown side by side, and both are fully editable: a change to the transcription automatically updates every linked report. A clinician can generate additional report types from the same consultation (for example a SOAP note and a referral letter from one session), and every report is explicitly marked as a draft until the treating clinician reviews, edits where necessary, and signs off.
